What do I need to bring to my DNA sample collection appointment?
The items needed for a DNA sample collection appointment are as follows, state-issued ID like a New York State driver’s license, passport, or any state-issued ID with a photo. The best ID for minors under the age of 18 is a birth certificate. DNA testing requirements:
If you are a New York State resident you will be required to obtain a doctor’s prescription, a letter from a New York State licensed Attorney or, a court order from a judge. Please note, if you decide to acquire a prescription it must be obtained from New York State licensed physician only. Prescriptions from an out of state doctor will not be accepted. The requirements stated are mandated by the New York State Department of Health.
